It doesn't really belong to "Will you love me when I'm old, fat and ugly?". At least it seems to me it indicates that the female in question actually IS old, fat and ugly at the time. So what works out of context does not necessarily have to do here. Plus it contradicts the "you can't actually escape it" note.
